Who is Eligible For Home Health Care?

Eligibility Conditions
Certain conditions must be met for persons to be eligible to receive services from our agency. The following is a list of these conditions:

1. Must be under the care of a doctor.
2. Must require intermittent skilled care.
3. Must reside in our agency's geographical areas.
4. Must have needs that can be safely and effectively met by our agency.
5. Must be essentially homebound.
6. Must have a doctor's order for home health care.
