| Scheduling, Messages, and Cancellations: |
 To
schedule an appointment for your child, please call during our office
hours. If you are making an initial appointment, you may be asked
to bring your child at a particular time. We have learned through
experience that certain procedures are accepted better by all in the
earlier part of the day. We make every effort to confirm appointments a day in advance and we require a 24-hour notice to cancel or reschedule an appointment. When necessary, messages may be left with our emergency answering service by calling our office number after hours at (904) 733-7254. |
| Our Payment Policy and Insurance: |
| We ask that you
pay for office visits and treatment at the time the service is rendered.
We accept most credit cards for your convenience. If you anticipate
a problem in paying for services, please discuss this in advance with
our receptionist. We will be pleased to help you prepare insurance claims for reimbursement, with the understanding that your insurance contract is between you and your insurance company and that you are responsible for all charges. Our itemized statement is readily accepted by most insurance companies when submitted with your claim form. Please remember to keep all receipts for insurance and tax purposes. |
